Job summary

Fairview Skilled Rehabilitation & Nursing Center is seeking a Limited Permit Doctor (LPD) to provide supervised medical care to residents under the guidance of a licensed physician. This role supports the medical team in delivering high-quality, person-centered healthcare in a skilled nursing and rehabilitation setting.

The LPD will participate in rounds, develop care plans with the supervising physician and IDT, and ensure documentation compliance with NYSDOH regulations.

Qualifications

• Graduate of an accredited medical school.
• Active Limited Permit issued by the New York State Education Department (NYSED).
• Eligibility for full New York State medical licensure.
• Strong clinical judgment and willingness to learn in a supervised setting.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct resident assessments, physical exams, and follow-up evaluations under supervision.
• Assist in diagnosing and treating acute and chronic medical conditions.
• Develop and implement care plans in collaboration with the supervising physician and interdisciplinary team (IDT).
• Monitor resident progress, update documentation, and address changes in condition promptly.
• Participate in rounds, care conferences, and interdisciplinary meetings.
• Complete all required clinical documentation in the EMR accurately and timely.
• Ensure compliance with NYS DOH, CMS regulations, and facility policies.
• Maintain supervision logs and licensure documentation as required for Limited Permit status.
• Communicate updates, treatment plans, and changes in condition to residents and families.
• Educate residents and families about diagnoses, medications, and procedures under supervision.
• Work closely with nursing, rehab, dietary, social work, and administration to deliver holistic resident care.
• Maintain high standards of ethical practice and professionalism.
• Participate in quality improvement initiatives and performance audits.
