Job summary

Fairview Skilled Rehab & Nursing Home is seeking a compassionate Limited Permit LPN for our nursing team. This role is designed for recent LPN graduates authorized to practice under a New York State Limited Permit while awaiting licensure.

You will work under supervision to provide direct care, administer medications, document care in the EMR, and support admissions and assessments within a collaborative interdisciplinary team.

Qualifications

• Recent practical nursing graduate
• New York State Limited Permit to practice as LPN
• Eligible to sit for NCLEX-PN examination
• Strong communication, organizational, and critical thinking skills
• Compassionate, dependable, and eager to learn
• Experience in skilled nursing/rehabilitation is a plus but not required

Responsibilities

• Provide direct nursing care under supervision of a Registered Nurse
• Administer medications and treatments per NY regulations and policy
• Monitor residents' condition and report changes promptly
• Document care provided in the EMR
• Assist with admissions, discharges, and resident assessments
• Collaborate with physicians, nurses, therapists, and other team members
• Ensure infection control, safety, and regulatory standards are met
