Job summary

Fairview Skilled Rehabilitation Center & Nursing Home in New York is seeking an Attending Physician (MD/DO) for on-site care of residents in a post-acute and long-term setting. The role involves clinical decision-making, supervising mid-level providers, and ensuring regulatory compliance within an interdisciplinary team.

The position supports flexible scheduling (full- or part-time) and requires a NY license with board eligibility or certification in Internal Medicine or Family Medicine;

Qualifications

• MD or DO from an accredited medical school.
• Active New York State Medical License; Board Certified/Board Eligible in Internal Medicine or Family Medicine.
• Geriatrics experience preferred but not required.

Responsibilities

• Provide comprehensive medical care to residents in skilled rehab and long-term care.
• Conduct initial evaluations, follow-ups, and rapid assessments of changes in condition.
• Oversee admissions, readmissions, and discharge planning; review orders and care plans.
• Collaborate with nursing leadership, rehab, social work, and administration.
• Supervise limited permit physicians, nurse practitioners, or physician assistants as applicable.
