Resources for Human Development (RHD) in Nazareth, PA seeks a Program Specialist – Lifesharing to support individuals with IDD and autism through person-centered planning and ISPs. The role focuses on monitoring quality, documenting services, coaching DSPs, and coordinating with families and partners across PA, ensuring regulatory compliance.

Qualifications

• Strong knowledge of Pennsylvania Chapters 6100 and 6500 regulations.
• Understanding of person-centered practices and Everyday Lives philosophy.
• Excellent organizational, communication, and problem-solving skills.
• Proficiency with Microsoft Office and basic computer systems.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Foster an environment that promotes independence, self-advocacy, choice, and person-centered planning for individuals with IDD and Autism.
• Support individuals in achieving goals and outcomes identified in ISPs.
• Monitor service quality to ensure health, safety, and well-being of individuals served.
• Conduct assessments and complete required documentation, including quarterlies and SEEPs, timely.
• Ensure services are documented in compliance with PA Chapters 6100 and 6500 regulations.
• Provide guidance, coaching, and ongoing support to Lifesharing Providers and DSPs.
• Facilitate training and educational opportunities for providers and staff.
• Collaborate with families, SCOs, AEs, ODP, and community partners.
• Serve as Incident Management contact and monitor incidents in EIM system.
• Participate in on-call rotation and provide after-hours support as needed.
