Job summary

Royer Greaves is looking for a Life Sharing Program Specialist based in Willistown Township, Pennsylvania. The successful candidate will coordinate and implement services to ensure individuals receive person-centered supports in accordance with Pennsylvania Chapter 6500 regulations.

Qualifications

Job description

POSITION: Life Sharing Program Specialist

DIVISION: Life Sharing

REPORTS TO: Executive Director

SUMMARY

The Program Specialist is responsible for coordinating, implementing, and monitoring services to ensure individuals receive person-centered supports in compliance with Pennsylvania Chapter 6500 regulations. This position oversees the assessment process, development and implementation of Individual Support Plans (ISPs), coordination of services, family support and training, and regulatory compliance related to individual rights, health and safety, medication administration, and residential services. The Program Specialist serves as a key member of the interdisciplinary team, ensuring individuals achieve desired outcomes while maintaining compliance with state and organizational requirements.

RESPONSIBILITIES

Individual Support Plan (ISP) Coordination and Implementation

Coordinate and complete assessments for individuals receiving services.
Ensure assessments are conducted and provided in accordance with Chapter 6500 regulations.
Participate in the development, annual updates, and revisions of Individual Support Plans (ISPs).
Attend and actively participate in ISP meetings.
Serve as the Plan Lead, when applicable, for ISP development, annual updates, revisions, and reviews.
Review ISPs, annual updates, and revisions to ensure content accuracy and regulatory compliance.
Report discrepancies or concerns regarding ISP content to the Supports Coordinator and plan team members.
Implement and monitor services outlined in the ISP.
Review the ISP with individuals as required and document completion of reviews.
Provide documentation of ISP reviews to the Supports Coordinator and plan team members.
Inform plan team members of their option to decline ISP review documentation.
Recommend revisions to services, supports, or outcomes when appropriate.
Monitor individual progress and ensure documentation accurately reflects participation and achievement of outcomes.
Supervise, sign, and date monthly progress documentation.
Report changes in an individual’s needs, health status, or circumstances to the Supports Coordinator and interdisciplinary team.

Service and Family Support Coordination

Coordinate services and supports provided to individuals.
Coordinate support services available to family members.
Provide or coordinate training for families regarding health, safety, and support needs specific to each individual.
Facilitate communication among service providers, families, and interdisciplinary team members to ensure continuity of care.

Program Development and Regulatory Compliance

Develop and implement provider services in accordance with Chapter 6500 requirements.
Ensure compliance with all applicable state regulations, organizational policies, and program standards.
Maintain accurate documentation and records required by state and organizational guidelines.

Physical Site, Health, and Safety Oversight

Monitor and support compliance with physical site requirements under Chapter 6500 regulations.
Ensure compliance with fire safety regulations and emergency preparedness requirements.
Promote and protect individual rights in accordance with applicable regulations.
Monitor health-related services and supports to ensure individuals receive appropriate care.
Oversee medication administration practices and ensure compliance with medication management regulations.
Identify and address health, safety, environmental, and risk management concerns.

Regulatory Knowledge Requirements

Working knowledge of Pennsylvania Chapter 6500 regulations, including:

Physical Site Requirements (§6500.81–84)
Fire Safety Requirements (§6500.101–110)
Assessment, ISP Development, Implementation, and Review Requirements (§6500.151–158)

QUALIFICATIONS

A Lifesharing Specialist shall have one of the following groups of qualifications:

A master’s degree or above from an accredited college or university and 1-year work experience working directly with person with intellectual disability
A bachelor’s degree from an accredited college or university and 2 years’ work experience working directly with persons with intellectual disability
An associate’s degree or 60 credit hours from an accredited college or university and 4 years’ work experience working directly with persons with intellectual disabilities
A high school diploma or general education development certificate and 6 years work experience working directly with person with intellectual disability

CERTIFICATES, LICENSES, REGISTRATIONS
