Resources for Human Development (RHD) is hiring a Program Specialist – Lifesharing in Pennsylvania to support individuals with IDD and Autism through person-centered services. You will ensure goal attainment, monitor quality, and maintain accurate records across multiple service line collaborations.

The role emphasizes training, compliance, and on-call support to empower communities. The Lifesharing program fosters independence and meaningful community participation with comprehensive training

Qualifications

• Strong knowledge of Pennsylvania Chapters 6100 and 6500 regulations
• Understanding of person-centered practices and the Everyday Lives philosophy
• Excellent organizational, communication, and problem-solving skills
• Proficiency with Microsoft Office and basic computer systems
• Valid driver's license
• Reliable transportation and ability to travel throughout assigned service areas
• Ability to maintain required clearances, certifications, and compliance standards

Responsibilities

• Foster an environment that promotes independence, self-advocacy, choice, and person-centered planning for individuals with IDD and Autism.
• Support individuals in achieving goals and outcomes identified in their ISPs.
• Monitor service quality to ensure the health, safety, and well-being of individuals served.
• Conduct assessments and complete required documentation, including quarterlies and SEEPs, within established timelines.
• Ensure services and supports are documented in compliance with Pennsylvania Chapters 6100 and 6500 regulations.
• Maintain organized and accurate individual records and program files.
• Provide guidance, coaching, and ongoing support to Lifesharing Providers and DSPs.
• Facilitate training and educational opportunities for providers and staff.
• Participate in interdisciplinary team meetings and collaborate with families, SCOs, AEs, the ODP, and other partners.
• Serve as the program's Incident Management contact and monitor incidents in the EIM system.
• Participate in an on-call rotation and provide support outside of regular business hours as needed.

Education

Master's Degree and one year of experience

Bachelor's Degree and two years

Associate Degree and four years
