Job summary

Access Services seeks a Lifesharing Coordinator to connect individuals with IDD to Lifesharing provider homes that match needs and goals. The role balances community-based work with administrative duties from the Bethlehem, PA area, with on-call responsibilities to ensure continuity of care.

The coordinator collaborates with individuals, families, providers, and community partners to deliver person-centered planning and maintain regulatory compliance while promoting independence and community

Qualifications

• Bachelor’s degree plus 2 years of experience supporting individuals with IDD and autism (IDD), OR
• Associate degree/60 credits plus 4 years of experience supporting individuals with IDD and autism (IDD), OR
• High school diploma plus 6 years of experience supporting individuals with IDD and autism (IDD)

Responsibilities

• Coordinate and oversee person-centered plans to support individuals’ social, emotional, physical, and educational needs.
• Match individuals with Lifesharing provider homes and support successful transitions.
• Train and support providers to ensure understanding of individual service plans and regulatory requirements.
• Monitor provider homes for quality, safety, and compliance with county and state regulations.
• Facilitate team meetings and collaborate with SCOs, AEs, and other community partners.
• Ensure timely and accurate documentation, including service notes, assessments, and required reviews.
• Connect individuals and families to community resources, supports, and respite services.
• Participate in on-call coverage and respond to after-hours needs as required.

Lifesharingis a unique program that allows individuals (18 and older) with intellectual disabilities tolive with qualified, trained providersand become a part of their lives and families.TheLifesharingCoordinator plays a key role in connecting individuals with intellectual and developmental disabilities toLifesharingprovider homes that best match their needs, preferences, and goals. Working collaboratively with individuals, families, providers, and community partners, this role supports person-centered planning, coordination of services, and successful integration into home and community life.This position ensures that services are delivered safely, compliantly, and in alignment with state and county regulations, while promoting dignity, independence, and meaningful community connection.

Homes arelocatedthroughout the Lehigh Valley region, with the primary office location based in Bethlehem, PA.This role balances community-based work and administrative responsibilities.

Note:This role includes participation in an on-call rotation and may require 15-20 hours of after-hours direct care support when needed to ensure continuity of care.

• Ensuretimelyandaccuratedocumentation, including service notes, assessments, and required reviews.
• Connect individuals and families to community resources, supports, and respite services.
• Participate in on-call coverage and respond to after-hours needs as required.

Theselected candidate must meet all qualifying and ongoing job criteria including background checks, physicalexamination,and a driving history record.

Driving Requirements:

• A valid US driver’s license for at least two years, not including time with permit.
• No serious traffic convictions within the past three years. (Serious convictions include, but are not limited to, DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations and/or at-fault accidents within thatperiod).
• Access to a reliable vehicle during work hours.Vehiclemust have valid registration and inspection stickers.
• Valid auto insurance. You will be asked to provide proof that you are a covered driver on the policy.

Education & Experience (one of the following):

• Bachelor’s degree plus 2 years of experience supporting individuals with intellectual and developmental disabilities and autism (IDD), OR
• Associate degree/60 credits plus 4 years of experience supporting individuals with intellectual and developmental disabilities and autism (IDD), OR
• High school diploma plus 6 years of experience supporting individuals with intellectual and developmental disabilities and autism (IDD)

Essential Working Conditions/Physical Demands:

• Workinprogram sites and the community routinely providing direct care and/or support withindividual(s).
• Stand,sitor run by self, orto provideproper care and supervision with anindividual.
• Able to move and stay with an individual to keepthemsafe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations.
• Able toassistan individualwho has fallen, regardless of weight or size.
• Walk, climb stairs by self, orto providedirect care and/or support.
• Workin a standard office environment.Utilize desktop computer equipmenton a daily basis.Able to lift or move items associated with sedentary work and light work (exert up to 20 lbs.) Travel to offsite locations and regional office locations if necessary.

Work Environment

• Lifesharingcoordinators work both in our Bethlehem office and the community.
• This role qualifies for hybrid remote work, subject to theAccess Services Hybrid Remote Work Policyand supervisor approval. Decisionsregardinghybrid remote work will be based on individual performance, program requirements, and the overall needs of Access Services.

Scheduleand Compensation

• The work schedule isgenerallywithinregular business hours(e.g., Monday-Friday,9AM-5PM) with flexibility to adapt to the needs of the caseload.
• Participation in a paid on-call rotation is required, which may include evenings or weekends as needed.
• Hourly ratestarts at $21.50/hour,based on education and experience.
