Job summary

Phipps Houses and Affiliates seeks a Life Skills Coach for the Lee Goodwin Residence in New York. The role provides comprehensive case management, outreach, and counseling to residents aged 18–28 who are aging out of foster care, with referrals to education, career programs, substance abuse treatment, and mental health services.

The position requires a Master’s or Bachelor’s in Social Work or related field, with experience in housing, health, and workforce services.

Qualifications

• Master's Degree and/or Bachelor’s Degree in Social Work, Psychology, Sociology, or Human Services or related field with two to five years experience preferred. Associate's Degree in related field is required.
• Previous experience in mental illness, substance abuse, housing, health, entitlements, workforce, and formerly incarcerated.
• Strong understanding of the developmental needs of young adults, sensitivity to ethnic and cultural diversity, and commitment to strength-based practice.
• Excellent written and verbal communication skills and strong computer skills.

Responsibilities

• Conduct a comprehensive intake assessment for all new residents and develop treatment plans; monitor emotional, social and physical well-being.
• Maintain concise and accurate resident files in Salesforce.
• Monitor residents’ medication.
• Provide residents with referrals to education, career programs, and other services.
• Participate in case conferences, staff meetings and trainings; represent the agency at meetings.
• Input data in Salesforce and Maven databases.
• Attend professional development trainings and supervision meetings.
• Provide monthly statistical reports on time and accurately.
• Work with staff to ensure quality service and improve team performance.
• Assist residents in planning, prioritizing, and pursuing long- and short-term goals.
• Maintain documentation for regulatory compliance.

Position Overview:

The Life Skills Coach at Lee Goodwin Residence provides comprehensive case management, outreach, and counseling to the residents at our supportive housing facility for young adults aging out of foster care between the ages of 18-28 year olds. The Life Skills Coach will provide referrals to: education and career programs such as: HSE, Vocational Training, Employment, and Internships. As well as: Substance Abuse Treatment, and Mental Health Services.

Salary: $55,000 to $58,000 (base salary commensurate with education, training and related work experience)

Responsibilities:

• Conduct a comprehensive intake assessment for all new residents and review the agency’s policies and procedures with them. Developtreatment plan for all residents. Monitor clients’ emotional, social andphysical well-being, as well as their progress
• Maintain concise and accurate resident files in Salesforce database
• Monitor residents’ medication
• Provide residents with necessary referrals, including emergencies suchas hospitalization, detox, etc. Inform residents about onsite, agencyand community resources that are available to them
• Participate in case conferences, staff meetings and training sessionsand represent the agency at assigned meetings and conferences
• Input data in Salesforce and Maven data bases
• Attend professional development trainings, supervision meetings, caseconferences, and staff meetings
• Provide monthly statistical reports on time and accurately
• Work cooperatively with staff at all levels to ensure quality serviceprovision and the development and improvement of team performance
• Assist residents in the planning, prioritizing and long-termgoals and short-term goals
• Maintain and oversee documentation of case files for compliance with regulatory agency

Qualifications:

• Master's Degree and/or Bachelor’s Degree in Social Work, Psychology, Sociology, or Human Services or related field with two to five years experience preferred. Associate's Degree in related field is required.
• Previous experience in the following areas: mental illness, substance abuse, housing, health, entitlements, workforce, and formerly incarcerated.
