StarCare Specialty Health System (StarCare) is seeking a full-time Enhanced Community Coordinator (ECC) - Service Coordinator to join its outstanding LIDDA team.
The Enhanced Community Coordinator (ECC) – Service Coordinator provides comprehensive coordination and monitoring services for individuals of all ages who are diverting from or transitioning out of a Nursing Facility or State Supported Living Center. This role supports a successful transition to community-based living by coordinating services, addressing barriers, and ensuring continuity of care. Enhanced Community Coordination includes ongoing monitoring and follow-up for up to one year after an individual's transition to the community to promote stability, independence, and long-term success in the least restrictive setting.
Responsibilities:
- Plans, coordinates, and monitors the delivery of services and supports for individuals on an assigned caseload, ensuring compliance with funding requirements and established protocols while promoting the effective integration of waiver and non-waiver services.
- Monitors the progress and status of individuals who have transitioned to the community within the previous year, documenting outcomes, satisfaction, and service effectiveness to ensure identified needs and authorized services are being met.
- Reviews and monitors the status of consumer services and supports by collecting and evaluating documentation related to incidents, emergency room visits, hospitalizations, increased physician visits, and temporary service suspensions.
- Provides individuals and/or Legally Authorized Representatives (LARs) with comprehensive information regarding community-based services, supports, options, and alternatives to support informed decision-making.
- Communicates program updates, service concerns, barriers, and other relevant information to supervisors, both verbally and in writing, to ensure continuity of care and effective service coordination.
- Completes and submits required documentation in accordance with departmental standards, funding source requirements, and regulatory guidelines, including Contact Notes, Service Plans, Diversion Plans, Transition Plans, and other required service coordination documentation and records as assigned.
- Maintain regular and consistent attendance for the assigned work hours.
- Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to StarCare policies and procedures.
- Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
- Support all activities in alignment with StarCare’s mission, values, and operational goals.
Schedule:
This is a full-time, on-site, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications:
- Bachelor’s degree in Human Services or a related field required.
- Minimum of two (2) years of experience providing service coordination to individuals with Intellectual and Developmental Disabilities (IDD).
- Bilingual proficiency in English and Spanish preferred.
- Experience utilizing SmartCare, TMHP, CARE, or similar electronic health record and service coordination systems preferred.
- Must have a current Texas driver’s license and be insurable under the Center’s insurance.
- Must pass all required pre-employment and annual screenings applicable to the position or services provided, as required by law, regulation, or agency policy, which may include criminal background, nursing registry, MVR, and drug screening based on applicable requirements.
