Description

StarCare Specialty Health System (StarCare) is seeking a full-time PATH Service Coordinator to join its outstanding SUD team. The PATH Service Coordinator serves as a frontline outreach and case management professional dedicated to supporting individuals experiencing homelessness or those at risk of homelessness who are living with serious mental illness and/or co-occurring substance use disorders. Through community-based outreach, engagement, assessment, service planning, and care coordination, this role connects individuals with critical resources, including mental health treatment, substance use services, housing assistance, healthcare, and other essential community supports. The PATH Service Coordinator establishes and maintains trusting therapeutic relationships, develops individualized service plans that promote stability and recovery, and ensures accurate documentation within a HUD-compliant case management system. Acting as a vital link between vulnerable community members and available resources, this position plays a key role in helping individuals achieve improved wellbeing, housing stability, self-sufficiency, and long-term recovery outcomes.

Responsibilities:
  • Conduct active outreach to unhoused individuals who aren’t yet connected to care, by meeting people where they are, including at encampments, shelters, or other community locations.
  • Conduct additional outreach activities by distributing program materials, participating in community events, and building relationships with local shelters, service providers, and community partners to increase awareness of and access to care.
  • Partner with clients to build individualized plans that reflect their goals, needs, and preferences and review progress with them regularly.
  • Help clients access housing, mental health care, substance use treatment, and other community resources.
  • Connect clients with primary health care, job training, educational opportunities, income support, and other needed services.
  • Complete required documentation and assessments to track client progress and ensure continuity of care.
  • Collaborate with multidisciplinary team members to ensure coordinated care.
  • Utilize trauma-informed, person-centered, and recovery-oriented practices when engaging clients and delivering services.
  • Ensure all services are delivered in accordance with organizational policies, professional standards, and applicable regulatory requirements.
  • Maintain regular and consistent attendance for the assigned work hours.
  • Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to StarCare policies and procedures.
  • Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
  • Support all activities in alignment with StarCare’s mission, values, and operational goals.
Schedule:

This is a full-time, on-site, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications:
  • Bachelor’s degree in social work, Psychology, Counseling, Human Services, or a related field required.
  • Minimum of one (1) year of experience providing case management, outreach, behavioral health services, homeless services, social services, or related support services preferred.
  • Experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, and/or co-occurring disorders strongly preferred.
  • Knowledge of community resources, housing programs, behavioral health systems, and recovery-oriented practices preferred.
  • Demonstrated ability to build rapport, establish trust, and effectively engage individuals from diverse backgrounds, including those who may be reluctant or resistant to services.
  • Strong verbal and written communication, organizational, time management, case documentation, and recordkeeping skills required.
  • Must have a current Texas driver’s license and be insurable under the Center’s insurance.
  • Must pass all required pre-employment and annual screenings applicable to the position or services provided, as required by law, regulation, or agency policy, which may include criminal background, nursing registry, MVR, and drug screening based on applicable requirements.
