Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our home-based care team. You will follow patient care plans, monitor progress, and collaborate with the RN Case Manager to adjust plans as patient needs change.

You will document medications, communicate with physicians and families, and help prevent unnecessary hospitalizations while delivering compassionate, high-quality care across patient homes.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds
• Must possess a valid driver's license or reliable transportation to travel between patient homes
• Experience in hospice/home health preferred

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Experience in a hospice or home health environment is preferred
