Job summary

Elara Caring seeks a Licensed Vocational / Practical Nurse to join our home health team in Oklahoma. You will follow each patient’s care plan, monitor progress, and update plans with the RN Case Manager to ensure safe, compassionate care at the patient’s home.

Requires an active LVN/LPN license, nursing diploma or equivalent, and at least 1 year of clinical experience. You must be able to lift up to 50 pounds and have reliable transportation to travel between patient homes.

• Graduate of an accredited nursing program.
• 1 year of clinical care experience.

As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
