Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to contribute to compassionate, high-quality care in a home-health setting in Indiana. You will follow care plans, monitor progress, and support patients and families with self-care techniques.

The ideal candidate has an active LPN/LVN license, a nursing degree or diploma, and at least 1 year of clinical experience; hospice or home health experience is preferred. Driving to patient homes may be required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift up to 50 pounds and assist with transfers.
• Valid driver’s license or reliable transportation to travel between patient homes.
• Hospice/home health experience preferred.

As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
