Medical Hill Healthcare in Oakland, CA is seeking a compassionate Licensed Vocational Nurse (LVN) for on-call/per diem coverage. You will provide direct nursing care, administer medications, monitor residents, and collaborate with CNAs, RNs, physicians, and the interdisciplinary team.

Formación

• Current and active California LVN license
• Current CPR certification or required
• Experience in skilled nursing/long-term care preferred

Responsabilidades

• Provide direct nursing care to residents according to care plans
• Administer medications and treatments as ordered
• Monitor residents' conditions and report changes to RN/physician
• Document care accurately and timely
• Assist with admissions, transfers and discharges as needed
• Communicate with residents, families and team members
• Maintain infection prevention and safety practices
