Qualifications

• California LVN license to practice in California.
• CPR certification required or preferred.
• Experience in skilled nursing or long-term care is a plus.

Responsibilities

• Provide direct nursing care per individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and report changes to the RN/physician.
• Perform nursing assessments and document care accurately.
• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with residents, families, and the care team.
• Provide wound care and other LVN scope treatments.
• Monitor vital signs and clinical indicators.
• Maintain timely and accurate medical records.
• Follow infection control procedures and policies.
• Respond promptly to emergencies and call lights.
• Ensure compliance with regulations and standards of practice.
• Promote dignity, safety, comfort, and quality of life.
