Job summary

Victorian Post-Acute in San Francisco, CA is seeking a dedicated Licensed Vocational Nurse (LVN/LPN) to support on-call coverage across weekdays and weekends. You will provide direct nursing care, administer medications, monitor residents, and assist with admissions, discharges, and assessments.

As part of a collaborative clinical team, you will supervise CNAs, coordinate with physicians, ensure documentation accuracy, and uphold safety and HIPAA standards in a resident-centered environment.

Qualifications

• Current, active California LVN/LPN license.
• Graduate of an accredited LVN/LPN nursing program.
• Experience in skilled nursing, post-acute care, or long-term care preferred.
• Strong clinical judgment, communication, and organizational skills.

Compensation: $36.00 - $38.00 per hour (based on experience)

Availability: Weekends and Weekdays as Needed

We are currently seeking a dedicated and reliable Licensed Vocational Nurse (LVN/LPN) to join our team on an on-call basis, covering both weekday and weekend shifts as needed.

As an On-Call LVN/LPN, you will:

• Provide direct nursing care and administer medications in accordance with physician orders and individualized care plans.
• Monitor resident conditions and report changes promptly to physicians, families, and the interdisciplinary team.
• Assist with admissions, discharges, treatments, and resident assessments.
• Support care planning and coordinate with healthcare professionals to ensure optimal resident outcomes.
• Supervise and guide Certified Nursing Assistants (CNAs) while promoting high standards of care.

Leadership & Documentation

• Direct and oversee nursing activities during assigned shifts.
• Maintain accurate and timely documentation, including charting, physician orders, treatment records, and care updates.
• Complete incident reports and participate in quality assurance initiatives.
• Collaborate with nursing leadership to ensure efficient operations and excellent resident care.

Compliance & Safety

• Follow all federal, state, and facility regulations, including HIPAA and infection prevention standards.
• Ensure proper medication administration, narcotic accountability, and treatment documentation.
• Promote a safe, clean, and comfortable environment for residents, visitors, and staff.
• Participate in required in-service training, facility surveys, and safety programs.

Qualifications

• Current, active, and unrestricted California LVN/LPN license.
