Job summary

Medical Hill Healthcare is seeking a compassionate LVN to join our team on an on-call/per diem basis in Oakland, CA. The LVN will provide direct nursing care, administer medications, monitor residents, and collaborate with CNAs, RNs, physicians, and the interdisciplinary team.

This on-call position offers AM, PM, and NOC shift opportunities, ongoing training, and a supportive leadership team. California LVN license and CPR certification are required or preferred, with skilled nursing experience

Qualifications

• Active California LVN license is required.
• CPR certification required or preferred.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide direct nursing care to residents according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and promptly report changes to the RN or physician.
• Perform nursing assessments and accurately document resident care.
• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with residents, families, and members of the interdisciplinary team.
• Provide wound care and other treatments within the LVN scope of practice.
• Monitor vital signs and other clinical indicators.
• Maintain accurate and timely medical records.
• Follow infection prevention and control procedures.
• Respond promptly to resident needs, emergencies, and call lights.
• Ensure compliance with facility policies, state and federal regulations, and standards of nursing practice.
• Promote resident dignity, safety, comfort, and quality of life.
