Job summary

Sweetwater Care in Oxnard, California seeks an LVN supervisor to provide nursing care within scope of practice, ensuring patient needs are met per standards, physician orders, and policies.

You will participate in quality improvement, educate families and patients, and administer medications and treatments as per facility policy. This role requires California LVN licensure, LTC experience, and familiarity with PointClickCare charting; management experience preferred.

Qualifications

• Currently licensed as LVN in state of California.
• One-year prior nursing management experience preferred, not required.

Responsibilities

• Provides nursing care within scope of practice to meet patient needs per standards, physician orders, policies, and guidelines.
• Demonstrates knowledge and implements the Infection Control Manual as part of daily practice.
• Participates in the Quality Improvement Program and quality life monitoring.
• Notifies departments of patient changes and coordinates admissions, discharges, and room changes.
• Conducts family and patient education as needed.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives and transcribes physician orders in accordance with Nurse Practice Act when allowed.
• Makes rounds with physicians as necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.
