Job summary

Sweetwater Care in California is seeking an LVN to provide nursing care within the scope of practice, following physician orders and center policies. This role emphasizes infection control, quality improvement, and patient education.

You will participate in education for patients and families, coordinate care with physicians and other departments, and ensure timely administration of medications and treatments. On-site, full-time position requiring LTC experience and a CA LVN license.

Qualifications

• California LVN license required.
• One-year nursing management experience preferred, not required.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• M akes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
