Job summary

Sweetwater Care is seeking an experienced Licensed Vocational Nurse in California for a nursing management role in a long-term care facility. The position requires strong leadership, familiarity with infection control, and the ability to administer medications and treatments per policy.

The role emphasizes participation in quality improvement, staff and family education, and collaboration with physicians. A compact license and PCC charting experience are preferred.

Qualifications

• Licensed LVN in California (or equivalent) with active license.
• Compact license if working in a Compact state.
• 1 year of nursing management experience preferred.
• Long-term care experience required.
• Point Click Care charting experience.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Utilizes Quality Life Manual for patient identification and monitoring.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
