Overview

Works under the supervision of a Registered Nurse. Duties include providing routine care, observing clients' health status, assisting the RN and physician in treatments, and communicating with clients and their families/guardians. Reports changes in condition or additional client needs to the supervisor.

Responsibilities

1. Communicate health/nursing needs and recommendations to RN supervisor.
2. Coordinate admissions, discharges, and follow-ups.
3. Assist during medical/health appointments as needed.
4. Provide nursing/health services under RN direction and physician orders, including direct care as per the Service and Nursing Care Plans.
5. Take and record vital signs, weights, wound measurements, etc., and report any abnormal trends.
6. Report health status changes to physicians and team members promptly.
7. Ensure availability of adaptive equipment.
8. Administer medications accurately, monitor client responses, and document accordingly.
9. Collaborate with pharmacy and healthcare staff on medication management.
10. Manage medication records, check supplies regularly, and dispose of unused medications per policy.
11. Report illnesses, communicable diseases, and work with team to maintain safety.
12. Document health status, care, and services accurately and timely.
13. Supervise and educate non-licensed staff in care provision.
14. Conduct quarterly nursing evaluations and refer assessment findings to physicians.
15. Record incidents, ensure safety, and communicate with RN and leadership.
16. Participate in clinics, on-call duties, and quality improvement activities.
17. Work hours may vary, including weekends and evenings.

Qualifications

• Completion of a state-approved training program (diploma, associate’s degree, etc.).
• Licensed Practical/Vocational Nurse (LPN or LVN) in good standing.
• CPR certification.
• At least 6 months of direct nursing experience.
• Experience with individuals with developmental disabilities preferred.
• Valid driver’s license with satisfactory record.
• Computer skills for documentation.
• Effective communication skills.
