Job Title: Clinical Quality LPN (Licensed Practical Nurse)
Work Location: Fully Remote
Work Hours: 8 AM - 5 PM EST/CST
Employment Type: Contract
Compensation: $29-30/hr
Job Summary: The Clinical Quality LPN is responsible for remotely accessing electronic health record (EHR) systems to conduct data mining and provider outreach for medical record collection and abstraction. This role supports HEDIS and STARs data gathering efforts, ensuring accurate documentation and compliance with quality standards. The LPN collaborates with other team members and the Supervisor of Quality Operations to facilitate successful data retrieval and abstraction.
Key Responsibilities:
- Maintain remote access to required EHR systems.
- Conduct data mining to close quality gaps within EHR systems.
- Contact provider offices to request medical records.
- Serve as a subject matter expert (SME) for HEDIS data collection and provider inquiries.
- Review and abstract medical record data into designated applications.
- Follow up with provider offices regarding incomplete or missing records.
- Analyze chart data for accuracy and completeness, documenting findings.
- Navigate multiple documentation systems to retrieve medical record sections relevant to HEDIS/STAR measures.
- Support hybrid chart chase processes by requesting records as needed.
- Stay up to date on HEDIS/STARs standards and guidelines.
- Perform other related duties as assigned.
Minimum Qualifications:
- Active Licensed Practical/Vocational Nurse (LPN/LVN) license.
- At least 1 year of experience in provider outreach and/or EHR system navigation.
- At least 1 year of experience with HEDIS/STARs data abstraction.
- At least 2 years of experience in data analysis and quality chart reviews, with the ability to assess medical record completeness.
- Strong oral and written communication skills.
- Proficiency in Microsoft Office applications, including Word, Excel, and databases.
Preferred Qualifications:
- Associate degree or higher.
- Certified Professional Coder (CPC) certification.
- At least 1 year of call center experience.
