Position Overview

Location: San Diego, CA
Must reside in California and have remote work experience.

We seek an experienced medical professional with a strong desire to deliver high-quality, exceptional care. Excellent communication, organization, and customer service skills are essential. The LVN collaborates with the interdisciplinary care team to coordinate care for complex patients, requiring critical thinking and organization. Our focus is on engaging and caring for medically complex patients, emphasizing empathy and a person-centered approach.

Compensation and Benefits

• Salary: $65,000 - $85,000 annually
• Paid time off: 3 weeks (including 2 weeks plus 6-7 federal holidays) and 401K
• Benefits: Medical, Dental, and Vision

Responsibilities

• Perform assessments, treatment planning, and ongoing care management using a person-centered approach.
• Make timely referrals for services such as social services, housing assistance, vocational rehab, mental health, medical, and substance abuse.
• Use motivational interviewing to engage and educate participants and coordinate follow-up care.
• Update registries, care plans, and clinical notes.
• Participate in care team meetings.

Qualifications

• 2+ years of experience as a Licensed Vocational Nurse.
• Unencumbered LVN license.
• Bilingual skills preferred.
• Remote work experience required.
