Job summary

Autumn Leaves Nursing and Rehabilitation Center is seeking a dedicated LVN-LPN or RN to join our full-time team in Texas. The role involves 12-hour shifts (6 to 6) and coordinating patient care with RNs and physicians, documenting changes, and preparing progress notes.

Candidates should have 1 year of LPN/RN experience, a valid compact or Texas nursing license, and CPR certification. We offer competitive pay and comprehensive benefits in a supportive work environment.

Qualifications

• 1 year experience as an LPN or RN.
• Compact license or Texas Nursing License in good standing.
• Proof of current CPR certification.
• Ability to work flexible schedule.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and summaries
• Assist the physician and Registered Nurse in complex nursing situations
• Follow universal precautions whenever giving any aspect of patient care
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition
