Job summary

The Springs Healthcare and Rehabilitation is seeking an LVN Supervisor to lead the nursing team on the night shift (10pm–6am). This full-time Texas-based role requires an LVN license and EMR experience to ensure nursing standards are followed and high-quality resident care is delivered.

You will supervise direct care staff, perform rounds, handle safety and infection control, and participate in shift reports and documentation. Competitive pay and comprehensive benefits are offered.

Qualifications

• LVN license in Texas is required.
• Previous supervisor experience in SNF preferred.
• Electronic Medical Record (EMR) experience required; PCC preferred.
• Must demonstrate knowledge to provide age-appropriate care.
• Be a supportive team member and promote teamwork.
• Ability to make independent decisions when warranted.
• Strong interpersonal skills with residents, families and staff.

Responsibilities

• Follow established nursing standards and implement programs, policies, and procedures.
• Obtain and relay shift reports to oncoming staff; document follow-up actions.
• Conduct daily resident rounds and report problems to supervisor.
• Identify safety hazards and initiate corrective actions.
• Provide emergency care per physician orders in the absence of a physician.
• Supervise direct care associates providing resident care.
• Oversee narcotics safety, counts and documentation.
• Notify physician of changes in conditions or abnormal results.
• Document daily notes for unstable residents and recently admitted residents.
• Ensure resident and staff safety and observe infection control procedures.
• Perform duties of a certified nursing assistant when required.
• Other duties as assigned.

What Is Expected

• Follow established standards of nursing practices and implement community programs, policies, and procedures.
• Obtain report from nurse being relieved and provide report to nurse coming on duty; record sufficient information to implement appropriate follow-up action, as necessary.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Identify safety hazards and initiate corrective action immediately.
• Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician.
• Provide supervision to direct care associates providing direct resident care.
• Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereof.
• Notify physician, when necessary, of changes in conditions, weight, medication errors, medication not available, abnormal lab/X-Ray results, and any other unusual observations
• Document daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residents.
• Be responsible for the safety of residents and associates under his/her supervision.
• Observe infection control procedures.
• Perform all the duties of a certified nursing assistant when required.
• Other duties as assigned.

Do I Qualify

• Unencumbered Licensed Vocational Nurse (LVN) License in Texas
• Previous Supervisor experience in SNF highly preferred
• Electronic Medical Record Experience required, PCC preferred
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
