Job summary

Autumn Leaves Nursing and Rehabilitation Center is seeking a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) for full-time, 12-hour shifts (6 a.m. – 6 p.m./p.m.).

You will participate in planning and coordinating total patient care, report changes in a patient’s condition, and assist physicians and RNs in complex nursing situations. Qualified candidates have at least 1 year of LPN/RN experience, a Texas nursing license (or compact license), and current CPR certification.

Qualifications

• 1 year of experience as LPN or RN.
• Valid Texas nursing license or compact license.
• Current CPR certification.
• Ability to work a flexible schedule.
• Strong interpersonal skills and communication abilities.

Responsibilities

• Coordinate total patient care with RN and physician according to the plan of care.
• Accurately report patient condition and changes in care, documenting signs and symptoms.
• Prepare clinical/progress notes and summaries.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions in all patient care activities.
• Follow emergency procedures for catastrophic events or significant patient changes.

Position and Schedule

• Licensed Vocational Nurse or Registered Nurse (RN)
• Full-Time
• 12 Hour Shifts (6 to 6)

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and summaries
• Assist the physician and Registered Nurse in complex nursing situations
• Follow universal precautions whenever giving any aspect of patient care
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition

Qualifications

• 1 year experience as an LPN or RN
• Compact license or Texas Nursing License in good standing
• Proof of current CPR certification
• Ability to work flexible schedule
