Job summary

The Springs Healthcare and Rehabilitation in Cedar Park, TX is seeking a dedicated LVN Supervisor for a full-time night shift (10pm–6am). This role includes supervising care staff, patient rounds, and reporting to leadership while ensuring safety and quality care.

Must hold a Texas LVN license and have strong EMR experience; SNF supervision is highly preferred. Competitive benefits and a supportive team environment are offered.

Qualifications

• Unencumbered LVN license in Texas.
• Supervisor experience in SNF preferred.
• EMR experience required; PCC preferred.
• Ability to make independent decisions when warranted.

Responsibilities

• Follow established nursing standards and implement policies.
• Provide shift reports to oncoming staff.
• Conduct resident rounds and report issues to supervisor.
• Identify safety hazards and initiate corrective action.
• Provide supervision to direct care staff.
• Monitor narcotics handling and documentation.
• Notify physician of changes in patient conditions.
• Document daily notes for unstable or new residents.
• Ensure resident and staff safety; observe infection control.
• Perform duties of a certified nursing assistant when needed.
• Other duties as assigned.

What is expected:

• Follow established standards of nursing practices and implement community programs, policies, and procedures.
• Obtain report from nurse being relieved and provide report to nurse coming on duty; record sufficient information to implement appropriate follow-up action, as necessary.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Identify safety hazards and initiate corrective action immediately.
• Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician.
• Provide supervision to direct care associates providing direct resident care.
• Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereof.
• Notify physician, when necessary, of changes in conditions, weight, medication errors, medication not available, abnormal lab/X-Ray results, and any other unusual observations
• Document daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residents.
• Be responsible for the safety of residents and associates under his/her supervision.
• Observe infection control procedures.
• Perform all the duties of a certified nursing assistant when required.
• Other duties as assigned.

Do I Qualify:

• Unencumbered Licensed Vocational Nurse (LVN) License in Texas
• Previous Supervisor experience in SNF highly preferred
• Electronic Medical Record Experience required, PCC preferred
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
