The Springs Post-Acute is a 99-bed Skilled Nursing Facility with a Post-Acute Rehabilitation setting, seeking a Licensed Vocational Nurse (LVN). The role involves direct patient care, medication administration, vital signs checks, and collaboration with an interdisciplinary team to support wound care, therapy services, and discharge planning.

Competenze

• Graduate of an accredited school of nursing.
• Newly graduated or Newly passers are welcome.
• Current California LVN license is required.

Mansioni

• Provide direct patient care under supervision of RNs or physicians.
• Administer medications, injections, and treatments as prescribed.
• Monitor and document vital signs, symptoms, and medical histories.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Assist physicians and RNs during examinations, procedures, and treatments.
• Perform wound dressing changes, catheterizations, and tracheostomy care.
• Document patients' progress and responses to medications.

RESPONSIBILITIES

• Provide direct patient care under the supervision of registered nurses or physicians.
• Administer medications, injections, and treatments as prescribed by healthcare professionals.
• Monitor and record vital signs, symptoms, and medical histories of patients accurately.
• Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans.
• Assist physicians and registered nurses during examinations, procedures, and treatments.
• Perform routine procedures such as wound dressing changes, catheterizations, and tracheostomy care.
• Monitor and document patients' progress, reactions to medications, and overall conditions.
• Educate patients and their families on disease management, medication regimens, and post-treatment care.
• Maintain a clean, safe, and organized work environment.
• Adhere to infection control protocols and maintain a high standard of hygiene.

QUALIFICATIONS/REQUIREMENTS:

• EDUCATION: Graduate of an accredited school of nursing
• EXPERIENCE:
  • Newly graduated or Newly passers are WELCOME
  • Experience Licensed Vocational Nurse (LVN)
• LICENSE/CERTIFICATIONS: A current California license as a Licensed Vocational Nurse (LVN) license is REQUIRED
