Qualifications

• Graduate of an accredited LVN program.
• Current nursing license in good standing with the State licensing board.
• Strong communication and organizational skills.
• Provide quality patient care per standards, policies and procedures.
• Identify and report problems; use initiative and good judgment.
• Knowledge of nursing principles to ensure care standards for residents.
• Willingness to work with professional/non-professional staff; English proficiency.
• Valid CPR certification.

Responsibilities

• Provides nursing care as prescribed by physician within scope of practice.
• Administers services and contributes to the nursing care plan.
• Admits/transfers/discharges residents per policy; orients new residents/families.
• Gives/receives nursing reports; ensures unit remains clean and safe.
• Conducts daily resident rounds and observes status.
• Records care information accurately; documents in medical records.
• Assists in developing and updating resident Plans of Care.
• Notifies physicians of significant changes and incidents.
• Prepares/administers medications and completes treatments.
• Participates in infection control and QA activities.
• Supervise CNAs.
