Job summary

Rockport Healthcare Services in California seeks a licensed practical nurse under the supervision of a Registered Nurse to provide nursing care for residents in a long-term care setting.

Responsibilities include admitting/discharging residents, conducting rounds, administering medications, documenting care, and contributing to the resident’s care plan while ensuring compliance with standards and policies.

Qualifications

• Graduate of accredited school of nursing or LVN program
• Current nursing licensure in good standing
• Strong communication and organizational skills
• Ability to provide quality patient care per standards
• Ability to identify and report problems with good judgment
• Knowledge of nursing principles to ensure quality of care
• English language proficiency (speaking, reading, writing)
• Valid CPR certification

Responsibilities

• Provides nursing care as prescribed by physician/health care professional within scope of practice and standards
• Administers professional services to help residents attain highest practicable well-being
• Admits, transfers, and discharges residents per policy
• Orients new residents and families as needed
• Gives/receives nursing reports at shift start/end and reviews daily patient notes
• Maintains safe, clean unit environment for residents
• Performs daily resident rounds and documents observations
• Manages medication administration and responses per physician orders
• Notifies physician/guardian of significant changes in condition
• Participates in care conferences and QA programs
• Supervises CNAs and supports nursing staff
• Records and documents care accurately in medical records
• Participates in infection control and safety programs
• Attends mandatory meetings and training
