Job summary

OhioGuidestone in Defiance/Napoleon offices seeks an office-based Licensed Therapist to support Outpatient Services across four counties. Duties include diagnostic assessments, treatment planning, therapy, and family support with options for telehealth.

Candidate must have a social work degree with LSW/LPC or higher (CDCA/LISW/LPCC a plus). Full-time or part-time hours are considered, with a strong commitment to community mental health.

Qualifications

• LSW or LPC or CDCA licensure required; LISW/LPCC a plus
• Bachelor's or Master's degree in Social Work or Clinical Counseling preferred
• Strong knowledge of behavioral and affective disorders with crisis response experience

Responsibilities

• Provide individual, family and group counseling and psychotherapy to assigned caseload.
• Develop and review Treatment Plans after the first encounter with client; document progress in notes.
• Maintain therapeutic relationships with clients, families and caregivers.
• Respond to mental health emergencies in a culturally responsive manner.
• Identify interventions for clients in crisis and coordinate with staff and community resources.
• Provide CPST, SUD TC and TBS care coordination as needed.
• Coordinate referrals and act as client advocate as needed.
• Attend supervision and document client/family interactions in EHR as required.
