Symetria Recovery in Illinois is seeking a licensed substance use counselor for our outpatient MAT-focused program. You will conduct assessments, develop individualized treatment plans, document progress, and lead individual and group counseling sessions.

You must hold LPC/LCPC/LCSW/LSW/CADC licensure in the state and have 1–3 years of experience, with proficiency in motivational interviewing, crisis intervention, and EMR systems.

Qualifications

• Licensed or certified as an LPC, LCPC, LCSW, LSW, or CADC in the state of employment.
• Knowledgeable of MAT/OTP regulations and treatment protocols.
• 1–3 years of experience in substance use counseling.

Responsabilités

• Conduct assessments of patients’ substance use history and needs.
• Develop individualized treatment plans aligned with goals and best practices.
• Maintain documentation of treatment plans, progress, and notes.
• Lead individual counseling sessions and group counseling.
• Equip patients with coping skills for long-term recovery.
• Create discharge plans with aftercare and relapse prevention.
• Coordinate referrals to medical and community services as needed.
• Deliver case management to ensure continuity of care.
• Advocate for patients to access required services.
• Schedule and track additional treatment or supportive services.
