Job summary

Symetria Recovery in Chicago, IL is seeking a licensed substance use counselor to join our outpatient MAT-focused team. You will conduct comprehensive assessments, develop individualized treatment plans, and document progress in EMR systems.

You will lead individual and group counseling, coordinate referrals, and advocate for patients while delivering case management to support long-term recovery. Illinois licensure or CADC is required, with 1–3 years of experience.

Qualifications

• Licensed or certified in Illinois as LPC/LCPC/LCSW/LSW or CADC.
• 1–3 years of substance use counseling experience.
• Knowledgeable of MAT/OTP regulations and treatment protocols.

Responsibilities

• Assess patients’ substance use history and treatment needs.
• Develop individualized treatment plans aligned with patient goals.
• Document treatment plans and progress in EMR systems.
• Lead individual counseling sessions and group counseling.
• Coordinate referrals to medical and community services.
• Deliver case management to address biopsychosocial needs.
• Advocate for patient access to services.
• Schedule and track additional treatments to support recovery.
