Job summary

Applegate Recovery is an outpatient addiction treatment program dedicated to high-quality, compassionate care for adults. We provide medically-supervised services and a trauma-informed approach to support recovery from opioid use disorder, including Medication Assisted Treatment where appropriate.

Qualifications

• Must hold LPCC/LICDC/LICDC-S licensure or LCDC II/III with supervision.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.
