Job summary

Applegate Recovery in Ohio is seeking a Licensed Counselor to support patients enrolled in MAT for opioid use disorder. The counselor will deliver individual and group counseling, collaborate with a multidisciplinary team, and help patients develop coping strategies to improve quality of life.

You will conduct initial assessments, establish treatment plans, respond to grievances, and contribute to aftercare and discharge planning, all while adhering to state regulations and trauma-informed

Qualifications

• Must have LPPC, LICDC, or LICDC-S licensure (or LCDC II/III with supervision).
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling; coordinate services per patient need and state regs.
• Conduct initial assessment and evaluation to determine eligibility.
• Complete bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints as needed.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.
