AppleGate Recovery is seeking a Full Time Licensed Counselor to support patients in Medication Assisted Treatment for opioid use disorder. The counselor will deliver individual and group counseling, participate in multidisciplinary care, and develop individualized treatment plans to improve quality of life.

The role includes collaboration with doctors, nurses, and a care team, with emphasis on trauma-informed practices and evidence-based methods.

Qualifications

• Must hold LISW, LPCC, LPC, AADC, or ADC licensure.
• Satisfactory drug screen and criminal background check.

Responsabilités

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.

Individualized coping techniques and symptom management skills that enable then to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.
