Job summary

Applegate Recovery, a leading outpatient addiction treatment program, is seeking a Full Time Licensed Counselor to support patients enrolled in Medication Assisted Treatment for opioid use disorder. The counselor will use trauma-informed, evidence-based practices to help patients build coping skills and improve quality of life, in collaboration with a multidisciplinary team.

Qualifications

• Must hold LISW, LPCC, LPC, AADC or ADC credentials.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.
