Qualifications

• College degree in Social Work or related field preferred and LI.
• Must have prior long-term care experience.
• Ability to make independent clinical and ethical judgments.

Responsibilities

• Evaluate patients based on psychosocial needs, strengths, and limitations.
• Communicate with patients, families, and case managers.
• Build relationships with patients and families to promote engagement in care.
• Coordinate care with interdisciplinary teams to support care planning.
• Develop and update individualized care plans reflecting resident needs and progress.
• Advocate for residents’ rights and access to services.
• Support discharge planning and transitions of care with external partners.
• Maintain timely documentation per policies and regulations.
