Job summary

Nashua Post Acute and Healthcare Center is hiring a Licensed Social Worker (LI) for a full-time, on-site role. This position focuses on counseling, care coordination, and advocacy to enhance residents’ dignity and quality of life.

Qualifications

• College degree in Social Work or related field and licensure.
• Prior long-term care experience.
• Ability to make independent clinical and ethical decisions.
• Strong knowledge of social work practices, laws, regulations, and guidelines for post-acute care.
• Ability to implement and interpret departmental programs, goals, policies, and procedures.
• Proficient in social services responsibilities, patient rights, safety, and sanitation.
• Adheres to Marquis Code of Conduct and completes compliance training.

Responsibilities

• Evaluate patients based on psychosocial needs, strengths, and limitations.
• Address patient concerns and goals while maintaining communication with patients, families, and case managers.
• Build and maintain trusting relationships with patients and families to engage care.
• Coordinate care with interdisciplinary teams to support comprehensive planning.
• Develop, implement, and update individualized care plans reflecting resident needs.
• Advocate for residents’ rights, emotional well-being, and access to services.
• Support discharge planning and transitions of care.
• Maintain accurate documentation per facility policies and regulatory requirements.
• Ensure compliance with applicable laws and standards governing social services.
