Qualifications

• Master’s degree in Social Work from an accredited institution.
• Licensed Independent Clinical Social Worker (LICSW). Licensure in NH may be required depending on territory and operational need.
• Five (5) years of related experience.
• CPR Certification is required upon hire if hired before 9/28/2026; if hired after, BLS Certification is required within thirty (30) days of hire.

Responsibilities

• Provides psychosocial assessment of the individual and family; develops treatment plans with appropriate interventions.
• Performs crisis intervention in situations involving suicide, violence, elder or child abuse.
• Assists with discharge planning and in-home care planning to ensure continuity of care.
• Provides treatment and counseling using individual, family or group modalities.
• Collaborates with health care teams to integrate psychosocial factors into care plans.
