Deerings Nursing in Texas is seeking a dedicated Social Worker to support residents’ social and emotional well-being. You will develop detailed social histories, craft care plans, and collaborate with an interdisciplinary team to ease transitions and crises.

We value reliability, empathy, and strong communication. A BSW or Social Worker Designee is required, with experience in long-term care preferred.

Formación

• Bachelor’s degree in Social Work or equivalent certification.
• Effective communication and interpersonal skills.
• Experience in long-term care settings.
• Compassionate, organized, and holistic well-being focus.

Responsabilidades

• Develop detailed social histories and assessments for residents and families.
• Create individualized care plans supporting goals and emotional well-being.
• Participate in interdisciplinary conferences and care planning.
• Provide individual and group support during crises or transitions.
• Assist with orientation and help with adjustments to the facility.
• Coordinate discharge planning for smooth transitions.
• Lead staff in-service training and social services education.
• Build relationships with external partners for referrals and care.
