Apploi is seeking a compassionate Social Worker in Lakeway, TX to support residents’ emotional and psychosocial needs and assist with discharge planning. You will work closely with residents, families, and the care team to promote quality of life.

Qualifications

• Current Texas Social Worker license required (LBSW, LMSW, or LCSW).
• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Experience in skilled nursing, long-term care, rehabilitation, or healthcare preferred.
• Knowledge of CMS regulations and long-term care standards preferred.
• Excellent communication, organizational, and interpersonal skills.

Responsibilities
