Job summary

Cross Healthcare Services seeks a compassionate Social Worker to support residents’ emotional and psychosocial needs and discharge planning within a multidisciplinary team. You will assess, counsel, and coordinate care aligned with CMS and facility standards.

Requirements include a Texas social work license (LBSW/LMSW/LCSW) and a BS or MS in Social Work. Prior long-term care experience is preferred; in-person work location.

Qualifications

• Current Texas Social Worker license required (LBSW, LMSW, or LCSW).
• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred.
• Knowledge of CMS regulations and long-term care standards is preferred.

Responsibilities

• Complete psychosocial assessments and develop individualized care plans.
• Provide counseling and emotional support to residents and their families.
• Coordinate discharge planning and transitions of care.
• Assist residents and families with advance directives, community resources, and long-term care planning.
• Participate in interdisciplinary care plan meetings.
• Advocate for residents’ rights and ensure resident concerns are addressed promptly.
• Maintain accurate, timely, and compliant documentation.
• Ensure compliance with CMS, state, and facility regulations.
• Collaborate with physicians, nursing staff, therapy, and other departments to coordinate resident care.
• Assist with admissions, transfers, and discharge processes as needed.
