Job summary

The Texas Health and Human Services Commission seeks a Social Worker III (licensed) for the Denton State Supported Living Center. This full-time, on-site role supports individuals with intellectual and developmental disabilities through social work assessments, care coordination, and advocacy.

You will lead admissions, transitions, and training, collaborate with interdisciplinary teams, supervise staff as needed, and ensure rights protection.

Qualifications

• Bachelor’s degree in social work and three years of social work experience, or, Master’s degree in Social Work.
• Licensed social worker (LBSW/LMSW/LCSW) required; LMSW or LCSW preferred.
• Experience with individuals with intellectual or developmental disabilities preferred.
• Ability to pass background checks and comply with agency policies.

Responsibilities

• Provide highly complex social work services and act as liaison between individuals, legally authorized representatives, and others to assure ongoing care.
• Coordinate admissions, transfers, transitions, and discharges with person-centered practices.
• Develop and deliver trainings to promote the exercise of rights and independence.
• Participate in interdisciplinary team meetings and case conferences as needed.
• Supervise and mentor social work staff as needed.
