Qualifications

• Bachelor’s degree in social work and three years of social work experience, or, Master’s degree in Social Work.
• One year of experience working directly with persons with intellectual or developmental disabilities.
• LBSW, LMSW or LCSW licensure required; LMSW or LCSW preferred.
• Applicants must pass a fingerprint background check and other pre-employment checks.

Responsibilities

• Performs and documents highly complex social services work and acts as liaison for individuals and representatives.
• Gathers information to assess support systems and needs and participates in interdisciplinary team meetings.
• Completes initial social history and maintains updates; may assess rights and provide training on rights.
• Assists with admissions, transfers, transitions, and discharges; supervises staff and students as needed.
