Job summary

Northern Light Health seeks a Licensed Social Worker for Home Care and Hospice in the Bangor/Houlton area. This full-time role operates 8:00 AM to 5:00 PM, with a salary range reflecting experience and location.

The position requires driving within the state and involves coordinating care with an inter-disciplinary team. The Social Worker will assess needs, connect clients with resources, provide counseling, and contribute to the treatment plan alongside physicians and care providers.

Qualifications

• Performs bio-psychosocial assessments to inform care planning.
• Provides counseling and linkage to resources for clients and families.

Responsibilities

• Collaborates with healthcare team to develop and evaluate care plans.
• Initiates ongoing comprehensive assessments of client and family needs.
• Demonstrates case management competencies and develops POC.
• Obtains authorization for deviations from agency protocols when needed.
• Documents accurately and adheres to standards of care.
• Provides patient-centered care and supports caregivers as needed.
