Job summary

Qualifications

Responsibilities

• Integrate management and delivery of comprehensive healthcare services.
• Perform clinical tasks within scope of duties and clinical expertise.
• Communicate information effectively to Providers, in written and oral form.
• Maintain confidentiality of patient, personnel and organizational data records.
• Coordinate with team to ensure smooth patient transitions and continuity of care.
• Provide education and linkage to community resources and short-term caregiver counseling.
• Attend education sessions to enhance or gain new skills.
• Meet productivity benchmarks and maintain a safe environment for staff and patients.
