UnitedHealth Group is seeking a Social Worker for Heart 'n Home Hospice, part of LHC Group, to provide medically directed social services with the Interdisciplinary Group. The role focuses on understanding social factors affecting health and aiding coping strategies.

The position requires a Master’s in Social Work, an OR Social Worker license, and access to reliable transportation. A sign-on bonus is available for external candidates.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• OR: Licensed as a Social Worker by the State Board of Social Work.

Responsibilities

• Assist patients and families to understand personal, emotional, environmental and social factors related to health status
• Contribute to development of a comprehensive Plan of Care with the health care team
• Instruct team on community resources to assist patients
