Job summary

Northern Light Health is seeking a Licensed Social Worker for Home Care and Hospice in Houlton, ME. The role is full-time, 8:00 AM to 5:00 PM, with salary range of $24.53–$37.65 per hour. Exempt status is No and a sign-on bonus is eligible.

The SW collaborates with the inter-disciplinary team to assess, educate and link clients to resources, supporting treatment and recovery in a home-based setting.

Qualifications

• Bachelor's degree required.
• Current driver's license in Maine with reliable transportation.
• Ability to travel within assigned region or statewide.
• License as LSW/LCSW/LMSW preferred.

Responsibilities

• Collaborates with health care team to provide integrative care and develops care plans.
• Performs comprehensive assessments of client, family and needs.
• Maintains documentation and evaluates client outcomes.
• Coordinates education and linkage to financial and community resources.
• Provides counseling to clients and short-term caregiver support.
• Confers with physicians to implement the home care plan.
• Adheres to clinical practice standards and agency policies.
• Participates in interdisciplinary team meetings and ensures care quality.
