Apploi is seeking a compassionate Social Worker in Lakeway, TX to support residents’ emotional and psychosocial needs and assist with discharge planning. You will work closely with residents, families, and the care team to promote quality of life.

The role requires a Texas Social Worker license and a degree in Social Work, with healthcare experience preferred. This is an in-person position with benefits and opportunities for professional growth.

Qualifikationen

• Current Texas Social Worker license required (LBSW, LMSW, or LCSW).
• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Experience in skilled nursing, long-term care, rehabilitation, or healthcare preferred.
• Knowledge of CMS regulations and long-term care standards preferred.
• Excellent communication, organizational, and interpersonal skills.

Aufgaben

• Complete psychosocial assessments and develop individualized care plans.
• Provide counseling and emotional support to residents and families.
• Coordinate discharge planning and transitions of care.
• Assist with advance directives, community resources, and long-term care planning.
• Participate in interdisciplinary care plan meetings.
• Advocate for residents’ rights and ensure concerns are addressed promptly.
• Maintain accurate, timely, and compliant documentation.
• Ensure compliance with CMS, state, and facility regulations.
• Collaborate with physicians, nursing staff, therapy and other departments.
• Assist with admissions, transfers, and discharge processes as needed.
